General Concrete Contractor

A company who has demonstrated compliance with the certification requirements and is able to document successful completion of at least fifteen cast-in-place concrete projects in the past five years. Company personnel shall have passed the ASCC examination on the ACI/ASCC Manual: “The Contractor’s Guide to Quality Concrete Construction.”

Decorative Concrete Contractor

A company who has demonstrated compliance with the certification requirements and is able to document successful completion of at least fifteen decorative concrete projects in the past five years. Company personnel shall have passed the ASCC examination on ACI/ASCC 310 “Guide to Decorative Concrete.”

Concrete Polishing Contractor

A company who has demonstrated compliance with the certification requirements and is able to document successful completion of at least fifteen concrete polishing projects in the past five years. Company personnel shall have passed the ASCC examination on ACI/ASCC 310 “Specification for Polished Concrete Slab Finishes.”

Specialty Expertise Endorsements

Two specialty expertise endorsements are available for Horizontal Construction (Slabs-on-Ground), and Vertical Construction (Suspended Slabs). The endorsements recognize the type of specialty area of concrete construction in which the company has additional expertise.

• Horizontal Construction (Slab-on-Ground): An ASCC certified Contractor Company with personnel who have passed the ASCC horizontal construction examination on ACI 302.1R-15 “Guide to Concrete Floor and Slab Construction”.
• Vertical Construction (Suspended Slabs): An ASCC certified Contractor Company with personnel who have passed the ASCC vertical construction examination on ACI 347.R-14 “Guide to Formwork for Concrete”.

Craft Worker Certification

In addition to the company certification and personnel specialty expertise, the company must employ craft workers with the following individual certifications:

• ACI certified Concrete Flatwork Finishers employed by ASCC certified General Concrete Contractor
• ACI certified Decorative Concrete Flatwork Finishers employed by ASCC certified Decorative Concrete Contractor
• CPC Craftsman Concrete Polishers and ACI Flatwork Associates employed by ASCC certified Concrete Polishing Contractor

Documentation Requirements for All Company Certifications - Phase I and II

1. List of Projects—at least fifteen projects in the last five years.
2. Letters of Reference-at least six references for the fifteen projects listed.
3. Safety Manual
4. List of Competent Person Designation—individual(s) responsible for safety.
5. Safety Metrics
   • OSHA Incident Rate— less than 6.5 in previous three years required
   • OSHA 300 and 300a— forms for last three years
   • Work Experience Modifier—average no greater than 1.1 in last three years required
6. Insurance Coverage
   • General Liability—a minimum of one million dollars ($1,000,000) combined single limit per occurrence for bodily injury and property damage and a minimum of one million ($1,000,000) completed operations provisions.
   • Workers Compensation and Employers Liability—as required by governing authority having jurisdiction.
   • Current Proof of Insurance—for General Liability and Employers Liability.
7. Quality Control Manual—including a quality assurance system of on-site inspections and checklists that promote the safe construction of a quality product.
8. List of Certified Personnel—ASCC and ACI/CPC certified personnel (see Craft Worker Certification).
9. Communications Plan— how information is communicated between the field, shop, consultants and other project team members.
10. Site Operations and Materials Storage Plan—defines operations on a current or past project site.

Site Audit - Phase III

The purpose of the site audit is to review actual documentation, observe and measure compliance with the program and the company’s stated procedures, policies and objectives. Documentation on at least two recent projects from the list of submittals for of the application should be available for the audit team to review. Documents will be made available for review by the audit team for inspection in order to verify compliance. The audit team personnel are not permitted to copy or remove any documents from the applicant’s premises without their permission.  The audit team will consist of a representative of the TCA and a non-competing contractor peer reviewer. The name of the peer reviewer will be made known prior to scheduling the audit to eliminate the potential for conflict of interest.
